Simple Community Health is a Community Care Management Organization (CCMO) which manages populations in the payor and provider community to specific outcomes. Our team has worked across the spectrum of healthcare, and regularly creates programs designed to blend superior care with operational and financial outcomes. Simple Community Health is a team of health care professionals who have taken their passion for patients and adopted the operational excellence found in the outsourcing and consulting industries to create an agnostic approach to healthcare outcomes.
Our team has worked with both payors and providers to create new care management models which meet challenges facing their communities. Leveraging this expertise Simple Community Health can create stand alone programs which allow health systems and independent providers to drive revenue while managed targeted populations; support payors in lowering annual cost by closing costly care gaps in underperforming populations, or bring the two sides together to create true value based care which shares common goals and lexicon.
Our Model
The core of Simple Community Health’s model is our care management team. This team is comprised of individuals who are trained community care navigators, RNs, LPNs, MSWs and LCSWs to provide comprehensive support as needed. Our nurse managed navigation and support is available 24/7 and is deployed based on each program’s parameters and each patient’s plan of care.
The navigation team is supported by technology and operational models which include the use of the Medicare Remote Patient Monitoring and Chronic Care Management programs, the program design of our team of physicians, and dedicated community partners in areas such as transportation, paramedicine, pharmacy and psychiatric services.
Simple Community Health deploys our model in a variety of environments as a full spectrum solution, or integrated into existing services as a support to achieve targeted outcomes.
